Most fortresses can spawn wither skeletons (and blazes) only in little cuboid-zones around the (location of the original, un-modified) fortress structure itself, but one fortress, the "special" fortress, can also spawn them on any nether brick block in the surrounding area. Only one would spawn per Nether Fortress, acting as a sort of mini boss like the Elder Guardian.
